Health Transitions Clinic at Physicians’ Clinic of Iowa is hosting a free introductory seminar at 7 p.m. March 14 in the Community Room at PCI Medical Pavilion, 202 10th St. SE, Cedar Rapids.
Those who are pre-diabetic, recently diagnosed with type 2 diabetes or struggling to control blood sugar, are welcome to attend. Interested individuals will learn more about Health Transitions Clinic, which has helped more than 500 people lose 15,000 total pounds and end their dependence on medications and insulin.
The 10-month program offers guidance from a physician and dietitian, monthly check-ins, support meetings and tools to track progress.
The process starts with an understanding of what dietary changes are necessary, implementing those changes, coupled with simultaneous changes in medications. Close monitoring of weight, blood pressure and blood sugar are necessary through this process. After initial weight loss, the team helps participants transition to an active lifestyle and develop a new diet they can maintain and enjoy.
